2. Fill the Credit Application out completely and Print it
3. Sign the Credit Application
4. Fax or mail the following items to us

a. Credit Application (filled out completely)*
b. Sales Tax Exception Certificate (Indiana Businesses only.)
c. Business License
d. If requesting to pay by credit card you must complete a credit card authorization form, please CLICK HERE for Details

This information must be sent to us, or your application will NOT be processed.
